Implanted Nerve Stimulation May Improve Health Outcomes in Sleep Apnea

More than 80 million U.S. adults live with obstructive sleep apnea, in which soft tissue at the back of the throat collapses and intermittently blocks airflow during sleep. The resulting drops in oxygen degrade sleep quality and increase the risk of diabetes and cardiovascular disease. Positive airway pressure (PAP) therapy is effective but often poorly tolerated, leaving many patients undertreated. A new study shows that hypoglossal nerve stimulation, a surgically implanted therapy, may offer downstream health benefits for these patients.
Hypoglossal nerve stimulation (HGNS) is a surgical system that connects an implant to the hypoglossal (tongue) nerve and pairs it with a breathing sensor. During inspiration, the implant stimulates the tongue nerve to protrude the tongue and prevent airway obstruction. The approach aims to achieve the same clinical goal as positive airway pressure therapy by maintaining airway patency, improving oxygenation, and restoring sleep quality. The study was conducted at the University of Chicago Medicine.
Investigators compared outcomes for nearly 3,400 adults who received HGNS with data from nearly 120,000 adults who had obstructive sleep apnea over the same time frame but could not reliably use a positive airway pressure device. Within two years, HGNS was associated with a lower likelihood of new diagnoses of diabetes or high blood pressure. These findings suggest a potential cardiometabolic benefit for patients who do not tolerate positive airway pressure.
The follow-up period was too short to determine definitive effects on long-term cardiovascular outcomes such as arrhythmias, coronary artery disease, or heart failure. However, the trends indicated decreasing risks for these conditions, and the work is described as the first to rigorously show an association between HGNS and prevention of hypertension and diabetes. The study was published in JAMA Otolaryngology–Head & Neck Surgery on July 16, 2026.
For clinical practice, the key message is that effective alternatives exist when positive airway pressure is not feasible. Patients who cannot tolerate devices may benefit from evaluation by a sleep-trained otolaryngologist to align therapy with anatomy and preferences.
"Studies demonstrate that if patients are not using PAP consistently and regularly—5 to 6 hours per night most days of the week—they may not see the benefits we demonstrate with HGNS in preventing downstream health risks," said Neil Kondamuri, M.D., an otolaryngology resident at the University of Chicago Medicine.
"If you feel your PAP machine does not help you feel rested or you just want to hear about alternative options, talk to a sleep-trained otolaryngologist about what other treatments might work specifically for your anatomy and preferences," Kondamuri said.
